Configure the whole arrival

More than custom fields: control when and how the questions appear

MemberMaker keeps the visitor experience simple while giving staff practical control over identification, question frequency, kiosk selection, and published versions.

Choose the visitor identifier

Use a normalized phone number or email address to recognize returning visitors. Collect name only when it is useful.

Ask every time or once

Run a prompt group at every arrival, or stop asking after the visitor completes the current required version.

Select the right kiosk experience

Switch between check-in only and any published prompt group. Apply it to one device or synchronize the choice to every kiosk.

One kiosk, different workflows

Keep several prompt groups ready. Switch when the job changes.

A weekday front desk, a special event, and a volunteer orientation do not need the same questions. Build each flow once, publish it, and choose the right group before the next visitor starts.

Common questions

Custom visitor sign-in form FAQ

Can I create more than one custom prompt group?

Yes. Create separate reusable prompt groups for different events, entrances, programs, visitor types, or workflows. Each group has its own identity settings, custom prompts, answer types, required fields, and question order.

Can I add custom questions to the MemberMaker visitor kiosk?

Yes. Add each custom prompt separately, choose its answer type and required status, add choice options when needed, then arrange the prompts in the order visitors should see.

What custom question types does MemberMaker support?

MemberMaker supports short text, long text, Yes or No, single choice, multiple choice, number, and date prompts. A single prompt group can mix these answer types.

Can I switch between prompt groups?

Yes. Choose check-in only or any published prompt group whenever the kiosk workflow changes. A device-specific selection can apply to one kiosk, or an organization-wide selection can synchronize to every kiosk. The new choice begins with the next visitor and does not interrupt a sign-in already underway.

Can visitors identify themselves with a phone number instead of email?

Yes. Each prompt group can use phone or email as its primary identifier. Name is optional, and you can choose whether to collect the other contact field.

Do custom visitor forms work offline?

Published prompt groups download to the kiosk device. Visitor check-in and answer collection can continue offline, then synchronize when connectivity returns. Creating, editing, and publishing forms requires a connection.

Can different kiosks use different visitor forms?

Yes. Staff can select check-in only or a published prompt group for one device, or apply an organization-wide selection that synchronized kiosks adopt.

Can I ask questions only once for returning visitors?

Yes. A prompt group can run at every check-in or only until a visitor completes the current required version. When publishing a revision, staff can decide whether existing visitors must answer again.
